Clinical Social Worker – Outreach and Housing
Title: Clinical Social Worker – Outreach and Housing
Reports to: Outreach Manager and Clinical Director

Description
The Clinical Social Worker will support St. John Center’s Outreach Team by providing behavioral health and clinical case management services to individuals and families experiencing homelessness. This position works collaboratively with Outreach Specialists and Housing Navigators to support unsheltered outreach efforts, diversion services, housing stabilization, and connections to community-based clinical supports.
The Clinical Social Worker will complete client intakes, biopsychosocial assessments, disability verifications, referrals to behavioral and mental health providers, Releases of Information (ROIs), and other clinical documentation necessary to support housing and treatment goals. This role will also support the St. John Center Diversion Program by assisting clients in identifying safe alternatives to shelter entry and strengthening natural support systems.

The Clinical Social Worker supports individuals and families experiencing homelessness through street outreach, housing navigation, and clinical intervention activities including:
- Building rapport and therapeutic relationships with unsheltered individuals experiencing homelessness
- Completing comprehensive intakes and biopsychosocial assessments
- Providing behavioral health support, crisis intervention, and emotional stabilization services
- Utilizing trauma-informed, harm reduction, and housing-first approaches in all client interactions
- Assisting clients in accessing medical care, mental health treatment, substance use services, benefits, and housing resources
- Completing Releases of Information (ROIs), referrals, and care coordination with community providers
- Supporting housing navigation efforts through clinical documentation and advocacy
- Completing disability verification forms and other documentation related to housing and benefits eligibility
- Supporting the St. John Center Diversion Program through assessment, mediation, and connection to community supports
- Advocating on behalf of clients and empowering clients to advocate for themselves
- Collaborating with hospitals, behavioral health providers, social workers, case managers, shelters, and other community partners
- Participating in multidisciplinary case conferencing and team meetings
- Maintaining accurate and timely documentation in HMIS and other required systems
- Communicating updates, concerns, and client needs with supervisors and team members
- Employing de-escalation techniques and responding appropriately during behavioral health crises
- Maintaining ethical standards, confidentiality, and professional boundaries in all interactions
Physical Requirements
- Conduct outreach visits in abandoned buildings, encampments, under overpasses, wooded areas, alleys, and other outdoor locations
- Maneuver safely through encampments and uneven outdoor terrain
- Sit, bend, lift light objects, walk extended distances, and climb stairs as needed
- Work outdoors in varying weather conditions including heat, cold, rain, snow, and icy conditions
Preferred Qualifications
- Experience working in homelessness services, behavioral health, outreach, crisis intervention, or community mental health
- Knowledge of housing resources, Medicaid services, and community behavioral health systems
- Experience completing clinical assessments and coordinating care across multiple systems
- Skilled in word processing, spreadsheets, databases, and electronic documentation systems
- Lived experience related to homelessness, mental health recovery, or substance use recovery valued
- Spanish language proficiency highly desired
Minimum Qualifications
- Certified Social Worker (CSW) credential required
- Eligible to bill Medicaid services
- Master’s degree in Social Work or closely related field preferred
- Strong communication, engagement, and interpersonal skills
- Ability to effectively engage clients regardless of circumstances or level of acuity
- Ability to de-escalate crises and work effectively in high-stress environments
- Valid driver’s license, reliable transportation, and liability insurance required
- Willingness to transport clients, co-workers, and operate agency vehicles
- Ability to work collaboratively in a fast-paced team environment while ad

Safety and Services Specialist – Sheehan Landing
Title: Safety and Services Specialist

Description
The Residential Peer Support Specialist at Sheehan Landing provides support and
mentoring for individuals in St. John Center’s single-site program. In tandem with other
members of the supportive housing team, the peer support specialist assists residents
achieve their self-determined goals while providing support with connection to
resources, The Residential Peer Support Specialist draws upon their lived experience to
provide a unique level of authenticity and understanding with clients while offering
insights to the team regarding best practices that align with Harm Reduction, Housing
First, Trauma-informed and Person-centered models of service delivery.
Responsibilities
- Monitor and Suport Property:
Collaborate with other staff to ensure property is safe sor residents, staff, partners, guest and volunteers. - Monitor Resident Flow and Safety:
Oversee the orderly entry, exit, and movement of residents within and around the property; actively monitor for safety and address any concerns as they arise. - Resident Engagement and Relationship Building:
Develop respectful, compassionate relationships with residents, staff and partners; communicate and support property expectations; engage in housing-related discussions and goal planning with case managers and residents. - Conflict De-escalation and Incident Response:
Intervene in resident disputes with calm and professionalism; de-escalate tense situations and manage behavioral incidents appropriately. - Provide Direct Social Services:
Support residents with essential services such as obtaining identification documents, transportation passes, and referrals to other programs. - Data Entry and Recordkeeping:
Accurately document services and resident interactions in multiple databases and resident files as required. - Coordinate External Services and Volunteers:
Liaise with partner agencies to facilitate on-site service delivery; welcome and assist volunteers; fill in for volunteer roles when needed (e.g., coffee service, hygiene distribution, phone coverage, check-ins). - Maintain Clean and Safe Environment:
Help ensure the property and surrounding property remain clean, organized, and safe; assist with the sorting and distribution of donations and material goods. - Support Staff Collaboration and Transitions:
Work with leadership and case managers during shift changes and support cross-program collaboration by sharing relevant client needs and updates. - Serve as Agency Representative:
Greet and guide visitors, donors, and guests; serve as a welcoming and informative presence on behalf of the organization.
Additional responsibilities include:
- Utilizing basic clinical skills to build rapport with clients and offering insights to the team regarding best practices.
- Functioning as a member of the team, contributing to daily needs, providing basic services to all-day to guests, facilitating group life skills instruction, and conducting intake assessments with new clients.
- Employing components of de-escalation daily.
- Completing necessary record-keeping in the Homeless Management Information System (HMIS) and completing other documentation as required by agency and specific situations.
- Addressing issues, problems, or questions of policy or procedure that may arise, defusing tensions, and communicating a shift or incident summary to the immediate supervisor and other staff members as needed.
Physical Requirements
- Ability to engage in standing or sitting for long periods, bending, light lifting, carrying, walking, or climbing stairs.
- Ability and willingness to safely and effectively de-escalate situations, including conducting physical interventions when necessary to maintain safety as directed by the agency.

Responsibilities
The Residential Peer Support Specialist provides positive support and helps formerly unhoused residents achieve housing stability through such activities as:
- Teaching and assisting residents with activities of daily living, and sustaining a new apartment
- Helping residents to locate and access community resources such as TARC, grocery shopping, food banks, employment, SNAP and social security benefits
- Accompanying clients to medical and mental health appointments as needed
- Initiating and engaging in conversations around harm reduction and recovery
- Accompanying clients seeking treatment and/or recovery support
- Assisting clients in creating and sustaining community connections
- Engaging in crisis intervention, supportive counseling, and life skills coaching

The Housing Peer Support Specialist is a member of St. John Center staff as a member of the housing team and will:

- Adhere to confidentiality guidelines and maintaining healthy professional boundaries

- Cooperate with community partners, including the Sheehan Landing Property Management team
- Represent clients and the agency appropriately in public
- Embrace principles of harm reduction and recovery
Physical Requirements
- Ability to travel throughout Metro Louisville to aid in apartment searches and perform home visits which may require ascending/descending full flight of steps, moving around in tight spaces, and other physical movement
- Standing or sitting for long periods of time, bending, light lifting, walking, climbing stairs
Minimum Qualifications
- High School Diploma or equivalent
- Basic computer literacy including word processing and Google suite
- Effective communication skills
- Must have personal transportation, valid driver’s license, liability insurance, and be willing to transport co-workers and/or client

- Completed the Kentucky State Peer Support Specialist Certification Training

Clinical Director

About SJC:
St. John Center serves individuals experiencing homelessness by addressing barriers to self-sufficiency and housing stability. Through outreach, day shelter, supportive services, and supportive housing, we provide trauma-informed care to individuals with complex mental health, substance use, and co-occurring disorders.
Position Summary
The Clinical Director provides organization-wide leadership for all behavioral health and billable clinical services across St. John Center’s programs. This role ensures high-quality, compliant, and sustainable clinical care for clients with high-acuity mental and behavioral health needs, including serious mental illness, substance use disorders, and chronic medical and psychosocial complexity.
The Clinical Director oversees clinical operations, diagnosis and treatment planning, credentialing, billing compliance, and Medicaid revenue performance, working closely with program leadership and the Director of Finance.
Key Responsibilities
Clinical Leadership and Direct Care
- Provide clinical oversight across all St. John Center programs.
Conduct diagnostic assessments, establish DSM diagnoses, and develop individualized treatment plans. - Coordinate care for clients with high-acuity behavioral health needs, including crisis intervention and co-occurring disorders.
- Lead multidisciplinary treatment team meetings and provide clinical consultation to staff.
- Ensure trauma-informed, harm-reduction, and evidence-based practices
Credentialing & Regulatory Compliance
- Oversee and maintain provider credentialing with Kentucky Medicaid, Medicare (CMS), IRISE, and other applicable payers.
- Ensure compliance with licensure, supervision, HIPAA, and regulatory requirements.
- Monitor clinical documentation to ensure medical necessity and payer compliance.
Billing & Revenue Cycle Oversight
- Oversee all billable clinical services and documentation.
- Complete or supervise prior authorizations and re-authorizations.
- Select, coordinate, and manage the external billing provider.
- Monitor claims, denials, and reimbursement trends.
- Partner with the Director of Finance to manage Medicaid reimbursements, revenue forecasting, and budget projections.
Systems, Quality & Staff Development
- Oversee EHR use to ensure data quality and audit readiness.
- Support clinical staff through supervision, training, and performance feedback.
- Promote staff retention and sustainable clinical workloads in a high-acuity environment.
Qualifications
Required
- Independent clinical licensure in Kentucky (LCSW or LPCC)
- Experience diagnosing and treating serious mental illness and co-occurring disorders
- Knowledge of Medicaid/Medicare behavioral health billing and documentation
- Strong clinical judgment and leadership skills
Preferred
- Experience serving people experiencing homelessness
- Prior clinical leadership or supervisory experience
- Experience with credentialing, audits, and Medicaid-funded programs
Compensation
- Base Salary: $70-$80
- Performance-Based Incentives: Total annual compensation may exceed $90,000–$100,000, based on:Base Salary: $75,000
