Supportive Housing Case Manager

Position Summary
St. John Center is seeking a highly organized, compassionate, and self-motivated Supportive Housing Case Manager to provide intensive case management services to individuals who have experienced chronic homelessness and face significant barriers to achieving housing stability. This position utilizes the principles of Housing First, Trauma-Informed Care, Person-Centered Planning, Motivational Interviewing, Strengths-Based Case Management, and Harm Reduction to support clients in obtaining and maintaining permanent housing.
The Supportive Housing Case Manager will maintain an active caseload of approximately 20–25 participants. Success in this role requires exceptional organizational skills, the ability to effectively prioritize multiple responsibilities, and a strong commitment to timely, accurate, and compliant documentation. The Case Manager will build meaningful relationships with participants while coordinating services that promote long-term housing stability, health, recovery, and self-sufficiency.
Responsibilities include, but are not limited to:
- Manage an active caseload of approximately 20–25 individuals receiving Permanent Supportive Housing services.
- Build trusting, professional relationships with individuals who have experienced chronic homelessness and may be living with mental health conditions, substance use disorders, physical disabilities, chronic medical conditions, or other significant barriers.
- Conduct comprehensive assessments to identify housing, medical, behavioral health, employment, income, and supportive service needs.
- Develop, implement, monitor, and update individualized service plans in partnership with each participant.
- Conduct regular home visits, with a minimum of one monthly home visit per participant, to assess housing stability, wellness, and service needs.
- Conduct outreach and engagement activities to support participants who may be difficult to engage or at risk of losing housing.
- Assist participants in locating, obtaining, and maintaining permanent housing while fostering positive relationships with landlords and property managers.
- Coordinate referrals and connections to community resources including healthcare, behavioral health, employment services, benefits, transportation, food assistance, recovery supports, and other community-based services.
- Provide advocacy and accompany participants to appointments when appropriate to remove barriers to accessing services.
- Utilize crisis intervention and de-escalation techniques while maintaining participant safety and housing stability.
- Facilitate life skills education, wellness activities, and community-building opportunities that support independent living.
- Monitor participant progress toward established goals and adjust service strategies as needed.
Documentation and Compliance
- Complete accurate, detailed, and timely case notes and required documentation within agency and funding source timeframes.
- Maintain organized client files and ensure documentation meets all HUD, grant, Medicaid (when applicable), CARF, and agency requirements.
- Document all participant contacts, assessments, service plans, referrals, home visits, critical incidents, and outcomes in the agency’s electronic health record (EHR).
- Effectively manage competing priorities while ensuring documentation remains current and audit-ready.

Position Summary
The Human Resources and Compliance Manager oversees all human resources, benefits, employee relations, workforce development, and compliance functions for St. John Center. This position partners with leadership to ensure compliance and employment regulations while supporting organizational effectiveness and employee engagement. The Human Resources and Compliance Manager manages the full employee lifecycle, including recruitment, onboarding, training, performance management, compensation, benefits administration, corrective action, retention, succession planning, and offboarding. The role also is an effective arm of CARF accreditation requirements, Medicaid documentation compliance, and 1915(i) RISE Program data management and reporting to support agency quality, compliance, and operational excellence
Human Resources Leadership
- Enhance HR strategies and initiatives aligned with the overall mission, vision, and strategic goals of St. John Center.

Qualifications
Education

Primary Responsibilities
- Overnight Client Support and Engagement: Build positive, professional relationships with residents during overnight hours by providing support, encouragement, and assistance with immediate needs. Foster a welcoming, trauma-informed environment that promotes dignity, respect, and stability.
- Resident Wellness Monitoring: Conduct routine wellness checks throughout the shift to ensure residents are safe and supported. Identify individuals who may be experiencing medical, behavioral health, or personal crises and respond appropriately

Description
The Clinical Social Worker will support St. John Center’s Outreach Team by providing behavioral health and clinical case management services to individuals and families experiencing homelessness. This position works collaboratively with Outreach Specialists and Housing Navigators to support unsheltered outreach efforts, diversion services, housing stabilization, and connections to community-based clinical supports.
The Clinical Social Worker will complete client intakes, biopsychosocial assessments, disability verifications, referrals to behavioral and mental health providers, Releases of Information (ROIs), and other clinical documentation necessary to support housing and treatment goals. This role will also support the St. John Center Diversion Program by assisting clients in identifying safe alternatives to shelter entry and strengthening natural support systems.

The Residential Peer Support Specialist at Sheehan Landing provides support and
mentoring for individuals in St. John Center’s single-site program. In tandem with other
members of the supportive housing team, the peer support specialist assists residents
achieve their self-determined goals while providing support with connection to
resources, The Residential Peer Support Specialist draws upon their lived experience to
provide a unique level of authenticity and understanding with clients while offering
insights to the team regarding best practices that align with Harm Reduction, Housing
First, Trauma-informed and Person-centered models of service delivery.
Responsibilities
- Monitor and Suport Property:
Collaborate with other staff to ensure property is safe sor residents, staff, partners, guest and volunteers. - Monitor Resident Flow and Safety:
Oversee the orderly entry, exit, and movement of residents within and around the property; actively monitor for safety and address any concerns as they arise. - Resident Engagement and Relationship Building:
Develop respectful, compassionate relationships with residents, staff and partners; communicate and support property expectations; engage in housing-related discussions and goal planning with case managers and residents. - Conflict De-escalation and Incident Response:
Intervene in resident disputes with calm and professionalism; de-escalate tense situations and manage behavioral incidents appropriately. - Provide Direct Social Services:

Responsibilities

About SJC:
St. John Center serves individuals experiencing homelessness by addressing barriers to self-sufficiency and housing stability. Through outreach, day shelter, supportive services, and supportive housing, we provide trauma-informed care to individuals with complex mental health, substance use, and co-occurring disorders.
Position Summary
The Clinical Director provides organization-wide leadership for all behavioral health and billable clinical services across St. John Center’s programs. This role ensures high-quality, compliant, and sustainable clinical care for clients with high-acuity mental and behavioral health needs, including serious mental illness, substance use disorders, and chronic medical and psychosocial complexity.
The Clinical Director oversees clinical operations, diagnosis and treatment planning, credentialing, billing compliance, and Medicaid revenue performance, working closely with program leadership and the Director of Finance.
Key Responsibilities
Clinical Leadership and Direct Care
- Provide clinical oversight across all St. John Center programs.
Conduct diagnostic assessments, establish DSM diagnoses, and develop individualized treatment plans. - Coordinate care for clients with high-acuity behavioral health needs, including crisis intervention and co-occurring disorders.
- Lead multidisciplinary treatment team meetings and provide clinical consultation to staff.
- Ensure trauma-informed, harm-reduction, and evidence-based practices
Credentialing & Regulatory Compliance
- Oversee and maintain provider credentialing with Kentucky Medicaid, Medicare (CMS), IRISE, and other applicable payers.
- Ensure compliance with licensure, supervision, HIPAA, and regulatory requirements.
- Monitor clinical documentation to ensure medical necessity and payer compliance.
Billing & Revenue Cycle Oversight
- Oversee all billable clinical services and documentation.
- Complete or supervise prior authorizations and re-authorizations.
- Select, coordinate, and manage the external billing provider.
- Monitor claims, denials, and reimbursement trends.
- Partner with the Director of Finance to manage Medicaid reimbursements, revenue forecasting, and budget projections.
Systems, Quality & Staff Development
- Oversee EHR use to ensure data quality and audit readiness.
- Support clinical staff through supervision, training, and performance feedback.
- Promote staff retention and sustainable clinical workloads in a high-acuity environment.
Qualifications
Required
- Independent clinical licensure in Kentucky (LCSW or LPCC)
- Experience diagnosing and treating serious mental illness and co-occurring disorders
- Knowledge of Medicaid/Medicare behavioral health billing and documentation
- Strong clinical judgment and leadership skills
Preferred
- Experience serving people experiencing homelessness
- Prior clinical leadership or supervisory experience
- Experience with credentialing, audits, and Medicaid-funded programs
Compensation
- Base Salary: $70-$80
- Performance-Based Incentives: Total annual compensation may exceed $90,000–$100,000, based on:Base Salary: $75,000
